Pair Team is a care coordination platform that connects Medicaid and Medicare patients with dedicated clinical teams and an AI health advocate named Flora to address medical, behavioral, and social needs. The service covers real-life support like housing, food, transportation, and mental health in addition to traditional medical care, all at no cost to eligible patients. The platform also helps community partners and clinics participate in healthcare programs to create sustainable financial support.
Pair Team assigns patients a dedicated care team of clinicians and social workers, supported by an AI health advocate (Flora) that coordinates appointments, resources, and escalations around the clock.
